University Health Services (UHS) is committed to providing the highest quality college health and wellness programs and services possible – and we need your help to do so!
You have a valuable role to play in helping UHS to improve our programs and services, and to determine the best use of our resources. We are asking that you complete the UMass Boston American College Health Association / National College Health Assessment (ACHA/NCHA). UHS will use what we learn from your responses to inform our programming, improve our services, and ensure that we are responding to the needs of our students.
To encourage as many students as possible to participate we will enter the name of each student who completes a survey into a raffle to win one of several prizes, including:
· ONE SEMESTER of instate tuition (sorry, not fees) or an Apple IPad of equal value (maximum of $850)
· $100 Bookstore / Dining Card
· $50 Bookstore / Dining Card
Participation in the survey is completely voluntary and is confidential. Your personal information will never be associated with your responses. You can choose to answer any or all of the questions, or you may choose not to participate at all.
